Cultivating an Effective Study Team through Application of Community-Engaged Research Principles

This session provided a case study of a research team to conduct a multi-site clinical trial with immigrant older adults and their caregiver dyads (PLAN trial).

Haera Han, PhD, BSN, RN, FAAN Bio: Dr. Han is a professor of nursing and public health at the Johns Hopkins University, and currently serves as Associate Dean for Community Programs and Initiatives at the School of Nursing. She is an expert on community-engaged intervention and methodological approaches to improve chronic care and outcomes for underserved people of color by improving health literacy. Her program of research has advanced thinking from the traditional paradigm of knowledge transfer from provider-to-patients to developing health literacy skills of patients to traverse the landscape of health screening and self-care to promote health equity. One of her ongoing research projects called PLAN—Preparing health aging through dementia Literacy education And Navigation—addresses dementia screening and linkage to dementia care in immigrant older adults, which will be discussed in detail from a community-engaged research team building and management perspective.  Learn more about Dr. Han here.

Irina Koyfman, DPN, NP-C, RN Bio (Co-presenter) Bio: Dr. Koyfman is a Nurse Practitioner and a Doctor of Nursing Practice with 25 years of nursing and 15 years of executive experience. Dr. Koyfman is an expert in the Patient-Centered Medical Home (PCMH), Home Health, Healthcare Start-ups, Transitional Care, Community Health, Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Care Coordination, and a Certified Dementia Care Specialist. Dr. Koyfman is a dedicated and enthusiastic clinician with an entrepreneurial drive making her a frequent presenter at multiple conferences. As a founder of Affinity Expert, a healthcare consulting company, she has been consulting primary care providers on all aspects of CCM, RPM and Dementia Care Planning.  She loves to give back and volunteers on multiple boards along with hands on volunteer work.
